WHAT TO BRING:
Your current state-issued driver's license.
Glasses, contacts, or hearing aids.
A list of all current medications you are taking.
Any special circumstances.
A pair of short to wear during your physical.
WHAT TO EXPECT:
Patients arriving for a DOT Physical will complete paperwork from the Department of Revenue, and will then be examined in vision and hearing tests. The Doctor will then examine the areas listed on the DOT Physical form. Patients should come well hydrated, as a urine specimen will be taken. Note - this is not for drug testing purposes. After a completion of the physical, a copy will be given to the patient and the employer (if needed). Patients passing the physical will be awarded a Medical Examiners Certificate for up to 2 years or less, depending on the physical results.

Group & Onsite Physicals
If you have a large group and want us to come to you, we offer Onsite Sports and School Physicals at a reduced price. For Salem Mo. and surrounding area schools or organizations who want to also use this for fundraising, we can charge a normal fee and part of the proceeds go to the school or organization. Call the office for more information or to schedule.
WHAT TO KNOW
Also called pre-participation physicals, sports/camp physicals are a great way to make sure an an athlete can safely play. Even if not required, sports physicals provide an opportunity to discuss any concerns you might have.
